 the nomination of its lead development candidate, REX-8756, an oral, reversible, and highly selective STAT6 inhibitor designed to address a wide range of type 2 inflammatory diseases, including atopic dermatitis.

The announcement coincides with the completion of Good Laboratory Practice (GLP) toxicology studies for the candidate, a milestone that initiated a $50 million payment under the company’s collaboration with Sanofi.

The development marks a step forward in targeting signal transducer and activator of transcription 6 (STAT6), a historically "undruggable" node in the IL-4 and IL-13 signaling pathway that drives many allergic and inflammatory diseases.

STAT6 is a critical transcription factor that mediates the effects of IL-4 and IL-13, 2 key cytokines in type 2 immune responses.

 Its activation leads to the transcription of downstream genes involved in inflammation, fibrosis, and immune dysregulation, all of which are implicated in diseases such as:

Chronic rhinosinusitis with nasal polyps 

Certain forms of chronic obstructive pulmonary disease

REX-8756: Novel Mechanism and Selective SH2 Domain Targeting

REX-8756 is designed to bind selectively to the Src Homology 2 (SH2) domain of STAT6, which plays a pivotal role in cytokine receptor signaling. Recludix has built proprietary DNA-encoded libraries and utilized custom screening assays to isolate small molecules that bind with high affinity and selectivity to STAT6's SH2 domain. Unlike protein-degrading approaches, REX-8756 inhibits STAT6 without degrading the protein.

Preclinical Efficacy in Atopic and Respiratory Disease Models

Recludix reports that REX-8756 exhibits complete STAT6 pathway inhibition in vitro and in vivo, durable target engagement without requiring continuous high plasma exposure, potent suppression of IL-4/IL-13-induced biomarkers, and efficacy in murine models of asthma, acute lung inflammation, and dermatitis.

Of particular interest to dermatology clinicians is the compound’s performance in dermatitis models, where it reduced skin inflammation and epidermal thickening. 

Recludix has also completed GLP-compliant toxicology studies for REX-8756, which showed favorable tolerability. The company has not reported any evidence of hepatotoxicity, immunosuppression, or hematologic dyscrasias, though full toxicology results are pending publication.

With investigational new drug (IND)-enabling studies underway, Recludix plans to submit an IND application later in 2025.

GLP-1s Emerging as Adjunct Tools in Psoriasis

, Ben Lockshin, MD, a board-certified dermatologist based in Rockville, Maryland, discussed the emerging role of glucagon-like peptide-1 (GLP-1) receptor agonists in dermatologic care, particularly in patients with psoriasis and obesity.

GLP-1 receptor agonists, including semaglutide and tirzepatide, are well-established for their efficacy in weight reduction. However, recent clinical interest is exploring their role in improving skin conditions, particularly when used alone or in combination with biologics in patients with moderate to severe psoriasis.

“There was actually a study that focused on using semaglutide as a monotherapy to improve skin in patients with PASI [psoriasis area and severity index] scores of 10 or greater,” said Lockshin. “We saw that there was not only weight reduction, but also skin improvement, as well as improvement in DLQI [dermatology life quality index] in those patients.”

While the majority of available data focuses on weight loss as the primary end point, ongoing trials are examining the added value of GLP-1 agents in dermatology. Notably, Eli Lilly is sponsoring 2 studies investigating combination therapy using tirzepatide (a dual glucose-dependent insulinotropic polypeptide [GIP]/GLP-1 receptor agonist [RA]) and ixekizumab (an IL-17A antagonist) in patients with elevated body mass index (BMI) and moderate to severe psoriasis.

These combination trials aim to determine whether reducing BMI can enhance the efficacy of biologics in psoriasis treatment. “We also have a blinded study looking at the combination of tirzepatide and ixekizumab versus ixekizumab alone,” Lockshin noted. “This could be a game changer for patients who haven’t responded optimally to biologics due to elevated BMI.”

Access to GLP-1 agents remains a challenge. Currently, patients typically qualify based on having a BMI ≥30 or a BMI ≥27 with a recognized comorbidity—psoriasis is not yet considered one. However, patients with commercial insurance taking ixekizumab may qualify for tirzepatide at a reduced cost through manufacturer-sponsored programs.

Lockshin emphasized the importance of multidisciplinary collaboration. “If you do not feel comfortable using a GLP-1, maybe your role is just recommending it to a patient, talking about psoriatic disease and encompassing many different comorbidities, including obesity,” he advised.

As research continues, GLP-1 RAs may soon be an essential part of managing chronic inflammatory skin diseases by addressing the metabolic factors that impact treatment outcomes and quality of life.

Road to RAD: What’s Next in the AD Pipeline?

, Jonathan Silverberg, MD, PhD, MPH, an associate professor of dermatology at the George Washington University School of Medicine and Health Sciences, shared insights into the rapidly evolving therapeutic landscape for atopic dermatitis (AD). He expressed enthusiasm about the robust drug development pipeline, which includes a wide range of topicals, oral medications, and injectable therapies currently in progress.

Among the most advanced candidates nearing approval are drugs targeting the OX40 and OX40 ligand pathway. These therapies represent a novel class distinct from the currently dominant type 2 cytokine blockers. According to Silverberg, the OX40 class of medications may have a slower onset of action and do not yet appear to outperform type 2 blockers in terms of immediate efficacy. However, they offer a notably longer duration of therapeutic benefit—what he refers to as a "long-tail" response.

This prolonged effect has generated interest in the possibility that these drugs might be disease modifying. Silverberg raises the question of whether patients who achieve skin clearance on these therapies could potentially maintain remission even after stopping treatment. Although these hypotheses remain unproven, they point to an exciting future direction in the management of AD, with implications for reshaping treatment goals and expectations over the next 5 to 10 years.

Beyond OX40-targeted therapies, Silverberg highlighted another promising frontier: multispecific therapies. Unlike current biologics that target a single molecule, such as IL-13 or the IL-31 receptor, new technological advances now make it possible to engineer therapies that can simultaneously block multiple cytokines with precision. This strategy could yield synergistic benefits, potentially improving efficacy without the broader systemic adverse effects often seen with less-targeted small-molecule drugs.

Silverberg emphasized that this multitargeted approach may significantly shape the future of immunologic therapy—not only in AD but also across a spectrum of immune-mediated diseases. He anticipates that this will be a major focus of research and development over the next 2 to 3 decades, underscoring a new era in personalized and durable treatment strategies.

Road to RAD: The Ins and Outs of Managing CHE

, Jonathan Silverberg, MD, PhD, MPH, associate professor of dermatology at George Washington University School of Medicine and Health Sciences, emphasizes the importance of accurately identifying the underlying etiology of chronic hand eczema (CHE). He noted that hand eczema can arise from a range of dermatological conditions, including irritant contact dermatitis, allergic contact dermatitis, and atopic dermatitis. Proper diagnosis is essential for effective treatment planning, and diagnostic tools such as patch testing can be especially helpful in distinguishing among these different causes.

When it comes to treatment, Silverberg acknowledges that topical corticosteroids are commonly used due to their affordability and widespread availability, especially in terms of insurance coverage. However, he points out a significant gap in the availability of approved nonsteroidal topical therapies for CHE. Currently, any use of nonsteroidal treatments is considered off-label, meaning these medications are not officially approved for this specific condition.

Despite this, clinicians often turn to a variety of topical agents in their therapeutic toolbox. This may include topical calcineurin inhibitors, which have long been used off-label for eczema management. Additionally, some newer topical treatments that have recently received approval for atopic dermatitis may be considered as options for patients with CHE. Nevertheless, Silverberg underscores that robust clinical data supporting the efficacy of these newer agents specifically for hand eczema are still lacking.
