Dr. Gregory House, from the hit television show, “House,” became famous for uttering the catchphrase, “It’s not Lupus!” This was in the context of him dismissing Lupus as a diagnosis to whichever medical mystery that was bestowed upon him and his fictional all-star team of specialist physicians.

Hershel Dobkin, MD, FAAD, despite being a former semi-okay-amateur-stand-up-comedian prior to being a dermatologist, sadly does not possess a mystery medicine television show. What he does possess is the experience of sometimes telling men that their genital skin area concern is not genital warts, also known as Condyloma Acuminatum, to those who enjoy proper medical terminology. And yes, of course both men and women can get genital warts, but in honor of men’s health month this commentary will focus on male patients, and the taboo around addressing their skin in sensitive areas.

Much of what a dermatologist does is reassure. There are numerous benign and lesser known cutaneous growths and anatomical variation that have designated diagnostic terms that can resemble Harry Potter spells, but nonetheless pose no medical threat to the patient. Polythelia -- the term for accessory nipple, Poikiloderma of Civatte -- skin changes on the neck due to longterm sun exposure, and Idiopathic Guttate Melanosis -- light spots on extremities from aging and longterm sun exposure, are all examples of diagnoses the non-dermatologist, when heard, may perceive as diseases, but are simply no immediate cause for concern.

Moving south or “inferior” in anatomical terms, we enter a differential diagnosis often clouded by emotions and embarrassment. In contrast to the “innocent until proven guilty” legal principle in our country’s justice system, genital concerns are “warts until proven not warts” to the vast majority of patients. Fleshy bump on genitals? Warts! Red bump on genitals? Warts! Occasional itch while staring down old men in the sauna? Warts! These are very real and common chief complaints men have presented with while concerned for genital warts.

It takes a great deal of vulnerability, courage, and overcoming of fear for many men to even mention a genital area skin issue, and as shocking as it may seem to the layperson, it’s a dermatologist’s job to treat this like any other skin complaint so as to prevent a “judgy” or awkward situation.

I am therefore granted the honor of reassuring many male patients, “It’s not genital warts.” Ironically, this reassurance that a patient’s perceived sexually transmitted infection is nothing but a benign growth or comparably harmless diagnosis creates a sense of emotional relief second only to telling a patient a lesion is not skin cancer. Nonetheless, that is what I and all dermatologists are here for.

There are more than a few notorious genital warts imposers that routinely cause unnecessary anxiety in men who come into our clinics. To expand on the “It’s not genital warts” differential, here are a few of the top offenders: To begin, we start with Pearly Penile Papules. These white papules straddling the penile corona are nothing but a type of angiofibroma-a benign growth of blood vessels and fibrous tissue. While we’re in the neighborhood, let’s discuss Tyson’s glands, yellow papules on the penis comprised of ectopic sebaceous (oil) glands. More subtlety different, the widely known diagnosis of acrochordons (skin tags) commonly masquerade as genital warts to the unseasoned condyloma eye (dermatologists). Lastly, and not as fleshy as the others, is the notorious Angiokeratoma of Fordyce-red or purple vascular red or purple papules often on the scrotum.

Once the emphatic relief from receiving a reassuring diagnosis passes on my male patient’s minds, there are a select few who ask the million-dollar question, “how do you know it’s not genital warts?” As with most other dermatological diagnoses, a dermatologist’s training and clinical experience is rooted in endless hours studying common and obscure dermatoses and pattern recognition in clinic settings across various skin types. The short answer to the above patient’s question would be, “Because I am a dermatologist,” but this might come across a tad bit pretentious. One day I may have enough dermatology “street cred” to answer this question with, “It’s not warts” while leaving the exam room like an 80s action hero walking away in slow motion from an explosion. The moral of this story is that a certain sensitive anatomical location shouldn’t hinder a concerned male patient from getting evaluated by a dermatologist -- it is all skin to us.

Dr. Hershel Dobkin, a dermatologist, emphasizes the importance of addressing male genital skin concerns without embarrassment, particularly during men's health month. Many benign conditions, such as Pearly Penile Papules and Tyson's glands, are often mistaken for genital warts, causing unnecessary anxiety. Dermatologists play a crucial role in reassuring patients by distinguishing these benign conditions from sexually transmitted infections. Their expertise is rooted in extensive training and pattern recognition. Encouraging men to seek dermatological evaluation for genital concerns is vital, as dermatologists view all skin issues with the same professional lens.

At RAD 2025, Christopher Bunick, MD, PhD, presented insights and clinical trial data on amlitelimab and rocatinlimab, highlighting their potential to suppress cytokine signaling, expand Tregs, and reduce memory T cells. 

OX40/OX40L Inhibitors Offer Upstream Signaling Pathways in Atopic Dermatitis 

Bunick positioned OX40 and OX40 ligand-targeting biologics within the broader therapeutic landscape, comparing them with existing options.

“Current mono-targeted biologics are sort of midstream in the signaling pathway, whereas JAK inhibitors are downstream,” Bunick noted.

OX40-OX40L therapeutics function at the interface between antigen-presenting cells and activated T cells. According to Bunick, by interfering with this co-stimulatory pathway, these biologics suppress the cascade of multiple cytokines contributing to AD’s pathogenesis, including its well-documented immunologic heterogeneity.

Beyond cytokine suppression, OX40/OX40L inhibition appears to induce broader immunoregulatory effects. Bunick highlighted the potential for these therapeutics to promote regulatory T cell expansion while reducing memory T cell populations, which may contribute to the persistence and recurrence of chronic atopic dermatitis.

Referencing his lab’s structural biology research, Bunick presented data showing the molecular distinctions between leading OX40/OX40L inhibitors: amlitelimab, rocatinlimab, and telazorlimab. He reviewed epitope-specific binding patterns and proposed mechanistic differences that may inform clinical behavior and future biomarker development.

Bunick also reviewed clinical efficacy and safety data among the OX40/OX40L therapeutics. He presented key findings from pivotal trials: phase 3 results for rocatinlimab and phase 2b data for amlitelimab. Both therapeutics demonstrated promising reductions in EASI scores and favorable proportions of patients achieving IGA 0/1 and EASI 75 end points, which were sustained through 52 weeks. Safety profiles were generally favorable, with no unexpected adverse signals, supporting their potential as well-tolerated long-term options for moderate to severe AD.

According to Bunick, OX40 and OX40L-targeting biologics represent a mechanistically distinct and promising addition for patients with atopic dermatitis. With their upstream targeting, immunomodulatory benefits, and emerging clinical data, they may address unmet needs, particularly in patients with refractory or heterogeneous disease. Bunick concluded by expressing excitement for ongoing studies and the potential for future combination or sequential therapies guided by mechanistic insights.

OX40/OX40L Inhibitors Offer Upstream Signaling Pathways in Atopic Dermatitis

Skin Improvement in Japanese Patients With Inflammatory Dermatologic Conditions Using the Injectable Hyaluronic Acid VYC-12

A retrospective study evaluated the use of VYC-12, a high–molecular weight hyaluronic acid injectable, in 15 adults with facial inflammatory skin conditions or aging-related concerns in Japan. Among 10 patients with treatment-resistant atopic dermatitis, acne vulgaris, or rosacea, VYC-12 significantly reduced skin redness and improved hydration over a 4-month period. All participants reported satisfaction with outcomes, and no adverse events occurred.

Prevalence, treatment and burden of chronic spontaneous urticaria in five European countries

A population-based study assessed the burden and treatment patterns of chronic spontaneous urticaria (CSU) across 5 European countries (France, Germany, Italy, Spain, and the UK). Mental and physical health scores were significantly lower in the CSU cohort than in the general population, with high rates of anxiety, depression, and health care resource use, particularly in the UK. Despite this burden, fewer than half of patients received prescription or over-the-counter treatment.

Mycoplasma-Induced Rash and Mucositis: Ocular Manifestations, Treatment and Outcomes in a Paediatric Population

A retrospective case series examined the ocular features and outcomes of mycoplasma-induced rash and mucositis in 14 pediatric patients. All patients exhibited conjunctival injection or ulceration, but no corneal involvement was reported. Most cases were managed conservatively with preservative-free antibiotics, lubricants, and selective use of topical corticosteroids under ophthalmologic guidance. Overall, 80% of patients recovered fully without long-term ocular complications.

Evaluation of routine care of prurigo nodularis in Germany: retrospective chart review study (ADVANCE PN)

A retrospective chart review (ADVANCE PN) analyzed medical records from 363 adults newly diagnosed with prurigo nodularis across 42 dermatologic centers in Germany between 2012 and 2022, revealing significant care gaps in routine practice. While nodules and papules were frequently reported, only a small fraction underwent patient-reported outcome assessments (8.8%) or disease-specific scoring (3.3%). Topical corticosteroids were the predominant first-line treatment, and in many cases, no second-line therapy was pursued.

Polynucleotides Enhance Skin Barrier Function and Reduce Inflammation in a 2,4-Dinitrochlorobenzene-Induced Mouse Model of Atopic Dermatitis

In a preclinical study using a DNCB-induced mouse model of atopic dermatitis, researchers found that topical polynucleotides (PNs) significantly improved clinical and immunological outcomes. PNs reduced dermatitis severity scores, spleen index, serum IgE levels, and transepidermal water loss, while histological analysis revealed decreased epidermal thickness, mast cell infiltration, and dermal inflammation. Additionally, PNs downregulated key inflammatory cytokines and preserved filaggrin expression in the skin. The combination of PNs with hyaluronic acid further enhanced therapeutic effects.
