 Antiperspirants containing aluminum chloride, like Drysol and Xerac AC, are often the first line of defense for those dealing with hyperhidrosis. When applied to dry skin before bedtime, they work by blocking sweat ducts and reducing excessive sweating.

 For localized hyperhidrosis affecting the face and head, prescription creams containing glycopyrrolate or wipes soaked in glycopyrronium tosylate (Qbrexza) can provide relief. These topical solutions are effective in reducing excessive sweating in the affected areas.

 Certain oral medications can be used to block the nerves that trigger sweat glands, reducing sweating in some individuals.

 Some medications typically used to treat depression may also help decrease sweating, particularly in cases where anxiety plays a role in exacerbating hyperhidrosis symptoms.

 Botox injections have gained popularity as a highly effective treatment for hyperhidrosis. By blocking the nerves responsible for activating sweat glands, Botox can provide relief for several months. 

Surgical and Other Procedures for Hyperhidrosis

 Iontophoresis is a home treatment method that involves soaking the affected areas, such as the hands or feet, in water while a device passes a mild electric current. This current blocks the nerves responsible for sweating. The treatment can be performed at home with a prescribed device.

 Microwave energy is used to destroy sweat glands in the armpits. The procedure typically requires two sessions, each lasting 20 to 30 minutes, and spaced three months apart.

 For individuals with localized hyperhidrosis in the armpits, sweat gland removal may be considered. This can be achieved through various methods, including scraping, suction, or a combination of both.

Hyperhidrosis management involves a range of treatment options, from topical and oral medications to surgical procedures. Prescription antiperspirants and creams are commonly used for localized sweating, while nerve-blocking medications and antidepressants can address systemic symptoms. Botulinum toxin injections offer temporary relief by inhibiting nerve signals to sweat glands. Surgical interventions, such as iontophoresis, microwave therapy, and sweat gland removal, provide more permanent solutions for severe cases. These treatments cater to varying degrees of hyperhidrosis severity and patient needs, offering both temporary and long-term relief options.

Beyond the Surface: Walter Liszewksi, MD, on New Frontiers in Chronic Skin Conditions

Liszewski, a board-certified dermatologist and associate professor of dermatology at the Northwestern Feinberg School of Medicine in Chicago, Illinois, shared insights into his current research and emerging dermatologic therapies. With a career devoted to managing patients with challenging itch and rash conditions, he emphasized the importance of consumer safety and advocacy in allergy research.

One of Liszewski’s primary research focuses involves patch testing for hair dye allergies, particularly those related to para-phenylenediamine (PPD) and toluene diamine sulfate, the 2 main hair dye allergens. His team is investigating how often patients sensitized to these allergens also react to oxidizing agents like resorcinol, which alter hair color shades. This work aims to better understand the epidemiology of cross-reactivity and improve diagnostic accuracy.

Additionally, Liszewski is examining the widespread use of lavender oil in baby products. While often marketed for its calming effects, he warns that high concentrations of lavender fragrance may contribute to developing fragrance allergies in children. Utilizing mass spectrometry, his research evaluates the concentrations of lavender molecules in commercial products to inform consumer safety recommendations.

Looking ahead to advancements in dermatology for the latter half of 2025, Liszewski is particularly enthusiastic about the evolving treatment landscape for chronic hand eczema (CHE). He noted that CHE is often multifactorial, involving atopic, irritant, and allergic contact dermatitis components, making it difficult to treat with current biologics targeting IL-4/IL-13 pathways alone. He highlighted topical and oral JAK inhibitors, including the recently approved delgocitinib, as promising therapies that address broader inflammatory mechanisms and offer new hope for patients suffering from debilitating CHE.

“I've had patients who are afraid to pick up their children because their hands are so cracked, they're afraid they're going to drop their kids,” he said. “The psychological toll and the financial toll from not being able to do the things you want to, to do your job is tremendous, and I'm so excited that CHE is finally getting some time in the spotlight.”

Liszewski also underscored the renewed attention on chronic spontaneous urticaria (CSU) within dermatology, encouraging clinicians to stay updated on emerging treatments that can rapidly improve patients’ quality of life without unnecessary delays.

Beyond the Surface: Walter Liszewski, MD, on New Frontiers in Chronic Skin Conditions

 from July 23 to 27, 2025, in Park City, Utah, new data reveals a positive shift in weekly Urticaria Activity Score (UAS7) bands after long-term treatment with remibrutinib in patients with chronic spontaneous urticaria (CSU).

Remibrutinib is a novel, highly selective, oral Bruton’s tyrosine kinase inhibitor. Previous data presented at the 2024 European Academy of Allergy and Clinical Immunology (EAACI) demonstrated the drug’s superior effectiveness when compared to placebo. The phase 3 global, double-blind, placebo-controlled REMIX-1 and REMIX-2 trials showed high efficacy and safety after 12 and 24 weeks, specifically in patients who remained symptomatic even after treatment with second-generation H1-antihistamines.

 Another REMIX analysis observed a reduction in CSU disease activity as early as week 1. This improvement was sustained up to 24 weeks in patients with moderate to severe disease.

The latest post-hoc analysis further explored the REMIX studies, assessing weekly UAS7 bands in patients being treated with both remibrutinib and placebo twice daily for up to 52 weeks from baseline.

Patients were randomized 2:1 to oral remibrutinib 25 mg bid (

= 306) for 24 weeks, followed by an open-label treatment with remibrutinib 25 mg bid for 28 weeks. Patients on placebo transitioned to remibrutinib at week 24. At baseline, 35% and 63.7% of participants taking remibrutinib had moderate and severe disease activity, respectively. In the placebo arm, 39.9% had moderate disease and 59.2% had severe symptoms.

CSU disease activity was categorized into five bands:

UAS7 > 0–6, Well-controlled disease activity

UAS7 = 16–<28, Moderate disease activity

Patients who treated with remibrutinib for the entire study saw substantial improvements in CSU symptoms, thus moving to a lower disease activity band as early as week 1. More patients remained in lower bands up to the 24th week. Patients who began with placebo and then transitioned to remibrutinib after week 24 then moved to a lower disease band and remained there, indicating no disease remission.

Approximately 63.7% of patients began the trial in the severe band. This dropped to 24.9% at week 1, 17.2% at week 2, 9.1% at week 12, 7.8% at week 24, and then 8.1% at week 52. Additionally, the 35.5% proportion of patients in the moderate band then dropped to 30.7% at week 1, 24.1% at week 2, 10.6% at week 12, 7.9% at week 24, and 7.3% at week 52.

Almost 12% of patients reached well-controlled or complete response at week 1. This rose to 31.5% after 2 weeks. In comparison, 0.7% and 4.2% of placebo-users reached the same response and weeks 1 and 2, respectively. By week 52, 35.1% of respondents had complete improvements. Patients who transitioned from placebo to remibrutinib then achieved similar band shifts.

Overall, patients with CSU who were treated with remibrutinib saw reduced disease activity as early as week 1 when compared to placebo. This fast response became long-term and improvement was sustained for the rest of the study. Additionally, patients who were originally treated with placebo and then switched reached similar levels of well-controlled and complete response by week 52. According to the authors, “remibrutinib has the potential to become a novel oral treatment option that provides fast (as early as week 1) and sustained improvements in disease activity in patients with CSU.”

Remibrutinib, a selective Bruton’s tyrosine kinase inhibitor, demonstrates significant efficacy in reducing chronic spontaneous urticaria (CSU) symptoms. In a study, patients treated with remibrutinib showed rapid improvement in Urticaria Activity Score (UAS7) bands, with many achieving well-controlled or complete response by week 1. This improvement was sustained over 52 weeks. Patients initially on placebo also experienced similar benefits after switching to remibrutinib. The findings suggest remibrutinib as a promising oral treatment option for CSU, offering fast and sustained symptom relief.

Remibrutinib Shows Rapid and Sustained Improvement in CSU Activity Score Bands

, like many in the community, is shocked and saddened to hear about the sudden passing of Robert Sidbury, MD, MPH, last Friday, July 11, at the age of 61. Sidbury was the chief of dermatology at Seattle Children’s Hospital, and a professor in the University of Washington's department of pediatrics. He was a proud member of the Society for Pediatric Dermatology (SPD), the American Academy of Dermatology (AAD), and Pediatric Dermatology Research Alliance (PeDRA), as well as a leader to many in the field.

“Dr. Sidbury was a visionary leader in pediatric dermatology whose career was defined by clinical excellence, groundbreaking research, and tireless advocacy,” said a joint release from SPD and PeDRA earlier this week. “He played a transformative role in shaping the field, particularly through his leadership in developing national treatment guidelines for atopic dermatitis. A prolific scholar, Dr. Sidbury authored numerous peer-reviewed articles on topics ranging from atopic dermatitis to pediatric skin infections and care of immunocompromised patients. His thoughtful approach to medicine, along with his generosity of spirit, made him a treasured mentor to many.”

In honor of Sidbury’s life, SPD will hold a special tribute at their upcoming meeting on Friday, July 25, titled “The Society for Pediatric Dermatology at 50: A View of our Specialty Through the Life and Work of Dr. Robert Sidbury.”

He was always curious and had a great love of learning, taking a few years to earn a Master's degree in Public Health and investigating the relationship between vitamin D levels and atopic dermatitis. He became a champion in teaching about the management and triggers of atopic dermatitis, co-Chairing the American Academy of Dermatology's Guidelines committee twice - in 2014 and currently. Above all he was incredibly giving - to his trainees, his colleagues, and his patients.
