In the final segment of her interview with Dermatology Times, Robin Smith, MD, MBA, reflected on the future of regenerative aesthetics and the expanding role of exosome technology in both skin and hair care. A longtime leader in the stem cell and regenerative medicine space, Smith is enthusiastic about the emerging science that bridges aesthetics and wellness, particularly the growing interest in biohacking and molecular-level interventions to preserve and restore biological pathways affected by aging, illness, or environmental stress.

Smith sees exosomes, peptides, biomimetics, and synthetic analogs as the next generation of active ingredients that will work in synergy to target specific physiological pathways, ranging from skin rejuvenation to hair growth. With the skin comprising about 15% of the body’s mass and serving as a home to hair follicles, maintaining skin health is not just cosmetic but crucial to overall well-being. As science advances, she believes patients will increasingly turn to evidence-based, non-invasive solutions to achieve results once only possible through surgery or transplants.

“So the more we can do to keep our skin healthy, to keep it hydrated, [to] prevent damage from the environment...[it] is going to be critical to not only the way we look, but our overall health.” Smith said.

Looking ahead, Smith envisions continued innovation in exosome engineering, combining specific types of exosomes for desired effects based on their unique molecular cargo and instructions. She stresses that these combinations are not random but based on a deep understanding of cellular communication and desired clinical outcomes.

Smith also noted a growing demand for transparency and sustainability. Today’s patients are more informed and expect skin care products to be not only effective but also ethically sourced and scientifically validated. With over nine years of dedicated research and development behind her brand Exoceuticals, her team has amassed a significant body of clinical data to support the efficacy and safety of their products.

“They're going to spend the money, but they want to know they're going to get the results that they're looking for...these are all things I think that are becoming more and more important to the consumer,” she emphasized.

Ultimately, Smith believes exosome-based technologies represent a powerful new era in regenerative aesthetics—offering customizable, high-performance solutions that are safe, sustainable, and deeply rooted in science. With continued innovation, she foresees exosomes playing an increasingly central role in dermatologic and hair care practices worldwide.

“We’re just very excited of what we've seen so far with skin care and hair care and just the ability to deliver improvement, with these topical products that are safe and efficient and efficacious,” Smith concluded. “We've done a lot of work over the last 9 years. We have a lot of data, a lot of understanding on the science, and we're just excited to keep growing and leading the industry.”
